At this time, I am an out-of-network provider and do not bill insurance directly. However, I am happy to provide a monthly Superbill that you can submit to your insurance company for potential reimbursement, depending on your specific plan’s out-of-network benefits.
Many insurance plans offer out-of-network mental health benefits that may allow you to receive partial reimbursement or apply payments toward your deductible. I recommend contacting your insurance provider directly to better understand your specific coverage and reimbursement options.

All mental health records requests must be submitted in writing, as required by Texas law.

Include your full name, date of birth, your therapist’s name, and the specific records you are requesting.
Can my records be sent to another provider or organization?
Yes. To release records to a third party, a signed Release of Information form is required before we can share any information.
Are there fees for records requests?
There may be a fee for records requests and document preparation, as permitted by law. Your provider can confirm whether fees apply.
What if records are requested for legal purposes?
A subpoena alone does not permit the release of confidential counseling or billing records under HIPAA. Records may only be disclosed with a valid court order or a properly executed Authorization from the client or parent or legal guardian.

What if consent has been declined?
If the client or parent has not consented to release of records, a Statement of Assurance will not be accepted. A valid Authorization or Court Order is required.
